Consent form for COVID-19 testing in secondary schools and colleges

Dear Parents/Carers,

 

 

I hope you all had a restful and enjoyable summer and that the children are looking forward to the start of the new term.

We are now able to confirm the arrangements for on-site lateral flow testing at the start of the Autumn term.

Testing is not compulsory, and students are not prevented from returning to school if they choose not to consent to testing.

 

In-school tests will be supervised by trained staff. Many of our students will already be used to the process, but we are aware that our new Year 7 children may need additional advice and guidance.

Students will swab themselves. The lateral flow tests are quick and easy to undertake, this time using a double nasal swab with results obtainable after 15minutes.

Anyone who has tested positive for Covid in the last 90 days is exempt from LF testing – however if new symptoms appear, they must follow the isolation and PCR testing rules.

 

Registration with NHS/HM Government

You will receive a text or email notification of a positive or negative result from the on-line NHS system by registering on the system within 24 hours of the test taking place.

Students will be given a registration card at the time of their test to enable you to do this.  The barcode sticker on the card is unique to your child’s test and this can be registered by using either the QR code or link – which are also on the card.  The test site ID is CHCS.

 

A positive result

If a student tests positive on a LF device, you will be contacted and arrangements must be made to have your child picked up immediately to self-isolate based on current public health guidance.  A further PCR test must be arranged in order to confirm the result.

 

A void result

If a student produces a ‘void’ result they will be recalled the same day in order to repeat the test.  Students may need to return to school to complete a retest if they are not on site.

 

A negative result

Parents will not be informed of a negative result by the school.  A negative result will be confirmed by registering with the on-line system described above.

 

Consent

Testing is entirely voluntary and if you do not wish/can’t consent to testing we also require you to visit the link to confirm that is the case so that we don’t make any unnecessary follow-up calls to confirm your position.  Testing is not compulsory, and you can withdraw your consent at any time by contacting the school office.

 

Please do not send your child into school for testing without preregistering.

 

Staggered start

To allow for the testing to take place we are having a staggered start to the term.  Please note the dates that are pertinent to your child/children.  We appreciate your support with this rather unusual start to the year.

Monday 6th September and Tuesday 7th September – staff INSET days.

 

Year 7

Their term will begin at 8.45am on Wednesday 8th September.  Yr7 Lateral Flow Testing will take place on the morning of the 8th September.

 

Year 8

Their term will begin at 8.45am on Thursday 9th September, having completed portal work from home on the 8th September.  Yr8 Lateral Flow Testing will be held on the morning of the 9th September.

 

Year 9

Their timetable will begin at 8.45am on Friday 10th September, having completed portal work from home on the 8th and 9th September.  However, please note that Yr9 Lateral Flow Testing will be held on the afternoon of the 9th September.  Students are invited in at the following times:

Thurs 9th Sept TESTING
1.00pm Aidan
1.30pm Bede
2.00pm Cuthbert
2.30pm Hild
3.00pm Oswald

Students must come into school 5 mins before their allocated time.  They must be in full school uniform and bring a pen.  Staff will be there to greet the children and direct them to registration and testing.  Students will return home immediately after the test has taken place – they do not need to wait for their results.

 

Year 10

Their timetable will begin on Friday 10th September having completed portal work from home on the 8th and 9th September.  However, please note that Yr10 Lateral Flow Testing will be held on the afternoon of the 8th September.  Students are invited in at the following times:

 1.00pm Aidan
1.30pm Bede
2.00pm Cuthbert
2.30pm Hild
3.00pm Oswald

 

Students must come into school 5 mins before their allocated time.  They must be in full school uniform and bring a pen.  Staff will be there to greet the children and direct them to registration and testing.  Students will return home immediately after the test has taken place – they do not need to wait for their results.

 

Year 11

Work Experience week begins on Monday 6th September and their timetable begins on Monday 13th September.  Students who are not on placement and working on the virtual Work Experience tasks are invited into school for testing on Friday 10th at the following times:

Fri 10th Sept TESTING
12.30pm Aidan and Bede
1.00pm Cuthbert and Hild
1.30pm Oswald

Students must come into school 5 mins before their allocated time.  They must be in full school uniform and bring a pen.  Staff will be there to greet the children and direct them to registration and testing.  Students will return home immediately after the test has taken place – they do not need to wait for their results.

 

Yr11 students who are on a Work Experience placement will be offered their 1st test on Monday 13th September.

 

Year 12 and 13

Timetables will begin on Monday 13th September.  However, please note that Yr12 and 13 Lateral Flow Testing will be held on Friday 10th September.  Students are invited in at the following times:

Fri 10th Sept TESTING
9.00am 12MSW, 12MSE and 12AMN
9.30am 12DGN, 12KGN and 12NFY
10.00am 13SRL, 13ANN and 13DTE
10.30 am 13PCD and 13CCS

The second test for all pupils will take place week beginning the 13th September.
